Feedback form on the checkout page

If your website has a checkout page after ordering, it could be relevant to implement a feedback form on this page. The following article will explain how you can do this.

Step 1: Set up your form

First, create your form with all the elements you would like and the desired design. An example of how your form could look like:

Whereby the second question only appears if the respondent selects any emoji except the ‘very happy’ emoji.

Step 2: Deploy

Next, add the form to your deployment and set the desired conditions. If the URL of your checkout page, for example, contains the word ‘payment’, you can set the condition ‘Show based on URL conditions’:
