Under the ‘Build’ tab > ‘Add’, you will find several different elements you can add to your form. This article will explain to you what each of the elements can be used for.

Scores

Rating:

This element is used to rate something, for example, you could use this if you want to know how your customers would score a certain product. This can be done with Stars, Numbers, Signs (-/+) and Emojis.

Net Promoter Score (NPS):

This element is used if you want to know how likely your customers are to recommend your company/product/service to someone else. This is on a scale of 0-10.

Customer Effort Score (CES):

CES is used if you want to know how much effort it took for a customer to execute a certain action. This is usually on a scale of “Very little effort” to “A lot of effort”.

Goal Completion Rate (GCR):

GCR is used if you want to know how often customers have completed something.

Thumbs:

This element is used if you want to know whether (and how often) a customer likes or dislikes something.

Question

Single Line Input & Text Area:

These two elements are used if you want your customer to write something themselves. The difference between the two is that for Text area, the customer can write more text.

Radio Button & Checkbox:

These are both multiple-choice elements. A customer can only select 1 answer with a radio button element and can select multiple answers with the checkbox element.

Contact details:

If you want to receive the name, e-mail and/or phone number from your customer.

Drop-down:

The answers in this element can be seen once you click on the drop-down. The customer can then select 1 answer.

Categories:

This is used if customers can choose themselves which topic to provide feedback on.

Screenshot:

If you want your customer to add a screenshot to elaborate more on an answer the customer provided.

Website Data:

This is used if you want to add additional data. This article provides you with more information on website data.

Likert & Matrix:

These two are handy if you have several questions with the same possible answers. Likert is used if you only have 1 set of answers, matrix is used if you have 2 sets of answers.

Display

Link & Multimedia:

This is used if you want to add a link or image to your feedback form for customers to view.

Section & Page Break:

Section break adds a space between questions on the same page. Whereas page break ensures that all answers before the page break are submitted when you go to the next page in the form. This article tells you a little more about page breaks.

Custom Front Page:

This is used if you want a separate front page.
