How to start?
Step 1
From the main menu on the left of the page, go to Data Collection > Feedback Forms.

Select an existing feedback form template to get started. To do this, click on ‘New form’.

You will then be redirected to a screen where you can select your feedback form template. Here you can select the form template that suits your needs.

By selecting your desired template, you will see a preview. It is also possible to edit the questions in the form template, or to add extra questions, by clicking on ‘Edit template in detail’.

Once you've selected your template and edited it how you want it to be, you can create your form by clicking ‘Save’ in the top right corner.

Click on ‘DEPLOY’ and then the pencil icon next to ‘Default deployment for web’. Select the survey you want to add to the deployment. Go back to the ‘DEPLOY’ page. Then all you need to do is grab the Mopinion code, by clicking on ‘<>’, and add and add it to your website, or add the form to an existing tag in the deployment section.
